What is Long Term Care?

Long term care residences—historically also referred to as nursing homes—are licensed and authorized as government-regulated and funded homes. They are designed for individuals who require the availability of 24-hour nursing care or advanced dementia support within a secure setting.

Ontario

If you are in your own home, LTC admissions are managed by local Home and Community Care Support Services (HCCSS) who will match you with a placement coordinator who will help you with your application.

If you are in hospital, the hospital will have a care coordinator (employed through HCCSS) who can help you.

Quebec

In Quebec, Chartwell does not operate in the long-term care sector, better known as centres d’hébergement de soins de longue durée (CHSLDs). These residential and long-term care centres cater to people experiencing an important loss of autonomy and fall under different categories: private CHSLDs, private CHSLDs under agreement, and public CHSLDs, which are funded by the Ministère de la Santé et des Services sociaux through the Financial Contribution Program by Accommodated Adults.

British Columbia

Also known asResidential Care, Chartwell offers both government-funded and private pay long term care in Nanaimo, Langley, Surrey and Burnaby. Applicable Regional Health Authorities in those locations coordinate admission and will perform an assessment to determine eligibility.
